Amounts Charged for Services if Paid Before or on the Date of Treatment:

15 Minute Massage + Chiropractic Adjustment/services = $35.00

Massage:

½ Hour = $40.00   45 Min. = $60.00   1 Hour = $75.00    1 1/2 Hours = $110.00

Add Chiropractic Adjustment/Services  to any Massage = + $20.00


Amounts Charged for Services if Credit is Extended to Clients under the following Circumstances:


Services Billed to Insurance
A Lien on Settlement accepted as payment
Any otherwise agreed upon Payment Plan


If credit is extended to a client and payment is remitted after the time of service, then the client will be billed in accordance with the medical service fee schedules located at http://www.cbs.state.or.us/wcd/policy/rules/accept.html.

Dr. Jeff did wonders for me! I am hiking the Pacific Crest Trail and stopped by for a sports massage and chiropractic adjustment. It was the best massage I have ever gotten. He relieved the pain in my shoulders, back and legs. It pushed the restart button on my trip! This was my first chiropractic adjustment, and I must say, I will be doing this again and again.
